Mayara dos Santos was riding a scooter when she was hit by a car and fell into an open manhole in Pará, Brazil on Wednesday.
A building surveillance camera showed the car driver making a left turn onto an avenue when the 27-year-old woman collided with the vehicle.
The impact of the collision sent her sliding across the road, before she fell head first into the drain.
Dos Santos spent four minutes trapped inside the manhole and was rushed to a local hospital.
She was diagnosed with spinal cord injuries and must undergo surgery
